Creditable Coverage Disclosure Notice
View the notice of Creditable Coverage for eligible Medicare Part D individuals enrolled in the Borough of State College's health plan with Capital Blue Cross. This notice is required for eligible subscribers to provide with their open enrollment for Medicare prescription drug coverage.
